Locust Fork names field house in honor of Hallmark



During the Tuesday, Sept. 4, Blount County Board of Education meeting, the board adopted a resolution to name the Locust Fork High School field house as the “Jeff Hallmark Athletic Facility.” The resolution states, “Whereas Jeff Hallmark spent much of his adult life devoting much of his time to the betterment of the Locust Fork High School athletic department; be it resolved, that the Locust Fork High School field house be designated as the Jeff Hallmark Athletic Facility.”

In other business, the board acted on the recommendation of Supt. Jim Carr and:

•approved payment in the amount of $27,737 for the Board of Education’s 2012 – 2013 Alabama Risk Management for Schools in General Liability/Errors and Omissions Liability Coverage.

•approved the following Federal Program plans for the 2012 – 2013 school year: LEA Improvement Plan, LEA Federal Programs Title I Plan, Title II Professional Development Plan, LEA Parental Involvement Plan, LEA EL Plan, LEA Homeless Plan, LEA Migrant Plan, Blount County Learning Center Corrective Action Plan.

•contracted with Kay Williams to provide homebound services to a sixth-grade student at the Blount County Learning Center. This instruction will begin on Aug. 20, 2012, and end upon the conclusion of the 2012 – 2013 school year. For this service, she will be paid $25 per hour plus mileage at the boardapproved rate from IDEA Part B and/or the general fund.

•contracted with Dr. Rhonda Poling to provide counseling to students attending the Blount County Alternative School beginning Oct. 1, 2012, and ending Sept. 30, 2013. she will be paid $250 per session, not to exceed a total of $5,000, from Title I FY2013 funds.

•contracted with Dr. Rhonda Poling to provide counseling to students referred to the Early Intervention Program beginning Oct. 1, 2012, and ending Sept. 30, 2013. Dr. Poling will be paid $100 per session, not to exceed a total of $10,000 from Title I FY2013 funds.

•contracted with Amy Lewis Gibbs to perform diagnostic/ achievement, intelligence and Autism diagnostic observation schedule (ADOS) evaluations beginning Sept. 4, 2012, and ending July 30, 2013. For this service, she will be paid a $35 per diagnostic, $90 per intelligence, and $110 per ADOS from IDEA Part B and/or general funds.

•contracted with Ronda Sullins to serve as a part-time guidance counselor at Locust Fork High School two days per week beginning Aug. 20, 2012, and ending April 30, 2013.

•contracted with Sharon Smith to serve as a JAG Specialist. She will work up to 50 days during the period beginning Oct. 1, 2012, and continuing through Sept. 30, 2013. Ms. Smith will be paid from the FY2013 JAG Grant.

•contracted with Angela Gardner to provide nursing services for students requiring medical care during extracurricular activities beginning Aug. 21, 2012, and ending July 31, 2013. For this service, she will be paid $25 per hour.

•contracted with Sonya Boatwright and Tammie Abernathy to provide nursing services for students requiring medical care during extracurricular activities beginning Sept. 4, 2012, and ending July 31, 2013. For this service, each will be paid $25 per hour.

•allowed Appalachian High School to contract with Mathew’s Manor in Springville for the 2013 Junior-Senior Prom on April 5, 2013. The total amount of the contract shall not exceed $3,500 and will be paid from the high school’s local funds.

•allowed Appalachian High School to enter into a contract with JAMM Entertainment to provide the musical services for the 2013 Junior- Senior Prom. The total amount of the contract shall not exceed $1,000 and will be paid for from the school’s local funds.

•allowed Cleveland High School to enter into a contract with Taylor Publishing Company for the production of the 2012 – 2013 yearbook. The high school is responsible for the entire payment.

•allowed Cleveland High School to enter into a maintenance agreement with Federalgraphics for a RISO Duplicator in the amount of $.005 per copy. The agreement shall begin on Oct. 1, 2012, and end on Sept. 30, 2015. Cleveland High School entered into another agreement with Federalgraphics for two NEC copiers owned by the high school in the amount of $.01 per copy. This agreement shall begin on Oct. 1, 2012, and end on Sept. 30, 2013.

•granted a leave of absence to five employees.

•employed Cassie Alldredge Washburn as a part-time caregiver on an as-needed basis for the Extended-Day Program at Southeastern School, effective Sept. 5, 2012. This employment is contingent upon need and revenue.

•employed Karen Blankenship as a part-time caregiver on an as-needed basis for the Extended-Day Program at Southeastern School, effective Sept. 4, 2012. This employment is contingent upon need and revenue.

•employed Shelia McAnnally as the bus driver for Hayden High School’s bus route to and from the Blount County Career Technical Center, and employed Lois Ann Mashburn as a utility school bus driver, both effective Sept. 5, 2012.

•employed Elizabeth Garnett as a long-term substitute teacher for Susan Moore Elementary School, beginning Sept. 5, 2012, and ending Dec. 18, 2012.

•paid Jackie Acton and Carla Bowens to work the admission gate at Hayden High School’s home varsity football games during the 2012 – 2013 school year. For this service, each will be paid $50 per game worked for a total of $250 each from Hayden High School’s local funds.

•paid Carla Washburn, a Child Nutrition Program employee, for approximately two additional hours per week during J.B. Pennington High School’s 2012 – 2013 varsity football season to prepare pre-game meals for the football team. Ms. Washburn will be paid at her regular hourly rate plus fringe benefits from the school’s football activity funds. If the additional work causes Ms. Washburn’s workweek to exceed 40 hours, she will be paid at time and onehalf for the excess hours.

•approved the personnel list for the 2012 – 2013 school year.

•approved the substitute teacher list for the 2012 – 2013 school year.

•approved the Child Nutrition Program/custodian substitute list for the 2012 – 2013 school year.

•approved to pay salary supplements for the 2012 – 2013 school year based on written requests from principals for athletic coaches, cheerleader sponsors, band directors, and persons assigned to maintain the athletic program fields during the summer months. These supplements are based upon the boardapproved supplement list and do not include supplements paid from local school funds.

•approved the requests made by principals to pay the following supplements for the 2012 – 2013 school year from local funds

•approved a field trip request for Appalachian High School’s ninth grade students to go to the Jr. Beta Leadership Conference, Panama City, Fla. for Nov. 3- 5, 2012

•moved Susan Hornbuckle, bookkeeper at Southeastern School, from the elementary bookkeeping salary schedule to the high school bookkeeping salary schedule, effective with the beginning of the 2012 – 2013 school year.

•employed Kelly Garrison Moon as a substitute teacher at Hayden Elementary School, effective Sept. 5, 2012.

•entered into a public works contract with Praters Athletic Flooring of Chattanooga, Tenn., for the renovation and refinishing of the gymnasium floor at Cleveland High School. The work shall be completed by Oct. 26, 2012. The total amount of this contract shall not exceed $12,700.

•accepted the resignation of Kelly Reynolds-Rizzo, a special education aide at the Blount County Learning Center, effective Sept. 14, 2012.
